About Welcome Home Food:

Sequim Food Banks “Welcome Home Food” program consists of pre-made meals along with fresh, ready to eat ingredients and snacks, delivered to your home for the first four weeks after being discharged from a hospital or medical facility. Food and medicine work hand in hand, making good nutrition an important part of care and optimal recovery. The pre-made meals have been created by a licensed registered dietician and are prepared at the Port Angeles Food Bank.

 

Eligibility:

·        A doctor or medical facility recommends Welcome Home Food to a client.

·        Client resides in Sequim or Port Angeles.

·        There are no income restrictions or requirements for this
program. It is a grant funded program and the client does not incur any costs.

 

How the Welcome Home Food Program Works:

·        Food boxes include six (6) pre-made nutrient dense, low sodium meals which are delivered to your home once a week for four (4) weeks.

·        At the end of four (4) weeks, Sequim residents can be added to a regular weekly food box
delivery schedule.

 

How to Enroll:

·        After receiving a recommendation from a doctor or medical facility, contact us at
